Overview

Welcome to the SSHC team. You have been hired as a caregiver. Caregivers may be eligible for benefits as either part-time or full-time employees based on hours worked.

Most Important Reminder Complete your benefit elections within the first 30 days of employment. If you do not enroll during your initial eligibility window, you must wait until the next annual open enrollment period unless you experience a qualifying life event.

Medical, dental, vision, life, and disability insurance may be available.

Benefit Eligibility & Enrollment

You may enroll in available benefit options, including medical, dental, vision, life, and disability insurance, provided you complete your benefit elections within the first 30 days of employment.

  • If enrolled on time, coverage becomes effective on the first day of the month following 60 days of employment.
  • If you do not enroll during your initial eligibility window, you will not be eligible to enroll until the next annual open enrollment period unless you experience a qualifying life event.
  • Benefit eligibility is based on hours worked and plan rules.

Work Hour Requirements & ACA Measurement Periods

Benefit eligibility for caregivers is determined based on average hours worked, in accordance with the Affordable Care Act (ACA).

Part-Time Caregivers

Must maintain an average of at least 20 hours per week. Part-time caregivers are eligible to enroll in available benefit options and are responsible for the full cost of elected benefits unless otherwise stated in plan documents.

Full-Time Caregivers

Must maintain an average of at least 30 hours per week. Full-time status is determined based on hours worked during the 11-month measurement period.

Eligibility Review

Eligibility is reviewed at the end of the measurement period.

Caregivers are expected to consistently accept and work available shifts to meet required hour thresholds. Failure to maintain required hours due to missed, declined, or canceled shifts within the caregiver’s control may result in loss of benefit eligibility.
Benefits will end in accordance with plan rules. The caregiver will need to wait until the next annual open enrollment period or experience a qualifying life event to re-enroll.
Absences protected by applicable law, including approved medical leave or FMLA, will not negatively impact benefit eligibility.

Employer Contribution

Caregivers determined to be full time under ACA guidelines are eligible for an employer contribution toward medical coverage.

Full-Time Caregivers Only SSHC will pay 50% of the employee-only medical premium during the applicable stability period.

Open Enrollment & Qualifying Life Events

Open enrollment is held annually during the month of November.
